What is a care plan?
When children and young people’s services get involved in looking after you they have to write down what they will do to support you and this is called a care plan. It says what children and young people’s services, your family and other agencies will do to support your health, education, religion and culture, contact with people who are important to you and your hobbies. Your care plan will be regularly reviewed to make sure that it is right for you.
