If you would like to discuss a child, young person or family at a SAM, you must obtain signed consent.

Consent should be recorded on a CAF assessment and will only be accepted on a FSM01 referral form in exceptional circumstances.  Both are available in the practitioners toolkit.

At least one week before the SAM you should register the signed CAF with the CAF team and ask to present at the next SAM (see contact details below).

You will then be contacted by the CAF team to inform you of the next SAM date.

All new cases can be presented by the practitioner working with the family or by the nominated SAM representative for that service with their agreement. 

If you are unable to attend, the CAF team will contact you after the SAM to share the outcome from your case.
